Phase I Activities (months 1-12)
- KDAP staff identify three or more entities within each participating county for the purpose of formalizing a local network, developing or supporting a county health council, and subcontracting with a non-profit or public entity to hire a local Community Encourager (CE).
- County Health Councils, with input from staff, hire CEs.
- CEs receive training in a modified Community-initiated Decision Making process (CIDM).
- Staff assist CEs throughout CIDM process: collect and evaluate health data, conduct surveys, inventory resources, promote community interest in health issues, market the CIDM process, and conduct community goal-setting meetings to identify and
